Joining Creation Calling for activities means you agree to be bound to our participant agreement posted below.
You will be asked to check a box marking your approval as part of each registration.
A parent or legal guardian must agree to the terms set forth in this agreement for each person to join us for classes and events.

CREATION CALLING / GLEN'S GIZMOS PARTICIPANT AGREEMENT

Applies to volunteers, teachers, staff and all participants.
Including emergency authorization, liability and photo release.

CHRISTIAN COMMUNITY: Many of the families who attend our events are part of the greater Body of Christ. Creation Calling is a christian ministry providing educational and memory making opportunities to strengthen families for Jesus. Teachers and event leaders must agree with and uphold our statement of faith and sign a Christian lifestyle agreement. Participating families do not have to be Christian to attend and join us for events and activities. You will be welcome and we hope you feel comfortable in our community regardless of your worldview or faith background. By attending Creation Calling sponsored events you are agreeing that you will not work against basic Christian principles found in scripture and summarized in our Statement of Faith. Intentionally acting or speaking against our statement of faith can be grounds for dismissal from our community.

LIABILITY RELEASE: I have familiarized myself with the Creation Calling's programs and events and understand that all activities are completely voluntary. I recognize the inherent risk of injury in group educational activities which may include, but not limited to: hiking on undeveloped trails, or no trails, archery, boating, fishing, crabbing, dance, gymnastics, physical education, water bottle rockets, chemical exposure and fire in labs, dissections, spending time around farm animals, animal diseases and bites, and a risk of catching contagious sickness from others.

If I, my family members, or other children I am attending with choose to participate in activities, classes, or volunteer, I hereby expressly assume the risk of injury or harm from any activities we participate in. I release all participating entities from all liability for injury, illness, property damage, or death resulting from the services I provide as a volunteer or occurring while I and my children are participating in activities or classes with Creation Calling.

I understand that Creation Calling, their host facilities and their volunteers/teachers have taken safety measures, including advanced planning and making efforts to aid the safety of all participants. However, I also recognize that Creation Calling their volunteers/teachers, and host facilities cannot insure or guarantee that the participants, equipment, grounds and/or activities will be free of accidents or injuries. I am aware of the importance of knowing and abiding by the rules and regulations shared at each activity and do release Creation Calling, their host facilities, their volunteers and the teachers from all liability for any injury to my my children, myself and other family members and any other children I am attending with and assuming responsibility for.

EMERGENCY AUTHORIZATION: In the unlikely event I or my second emergency contact cannot be reached in a life threatening emergency, I give permission to the physician selected by Creation Calling or their representatives to hospitalize and secure proper treatment for my family members or any children I am attending with. *This permission does NOT include administration of immunizations or vaccinations which are not emergency care and still require the additional consent of a parent/guardian.

PHOTO AND VIDEO RELEASE: I give permission for Creation Calling, the host facility and the volunteers/teachers to use any photo, video, or interview taken of myself or my family members to be used to illustrate, report, promote and advertise through in person events, online at websites, TV broadcasts, streaming services and through social media at the discretion of Creation Calling staff and volunteers.
